Welcome to Monarch Ideal Care. These Terms of Service ("TOS") outline the terms and conditions under which we provide medical care and services. By accessing or using our services, you agree to be bound by these terms. If you do not agree with these terms, please do not use our services.

1. Services Provided

Monarch Ideal Care offers a range of medical services, including but not limited to primary medical services, laser hair removal, hormone optimization. Our services are designed to provide high-quality healthcare to our patients in accordance with professional medical standards.

2. Patient Responsibilities

As a patient of Monarch Ideal Care, you agree to:

Provide accurate and complete medical and personal information.

Follow the medical advice and treatment plans provided by your healthcare provider.

Inform us promptly of any changes to your health, medications, or personal information.

Make timely payments for all services provided.

Failure to follow these responsibilities may result in termination of care or treatment delays.

3. Appointments and Cancellations

Patients are responsible for scheduling and attending their appointments on time. If you need to cancel or reschedule an appointment, we require at least 1 hours' notice. Appointments canceled with less notice, or failure to attend an appointment without notice, may result in a no-show/late cancellation fee of $50.

Repeated failure to attend appointments or late cancellations may result in termination of services.

4. Payment and Billing

You are responsible for all payments associated with the services provided by Monarch Ideal Care. Payment is due at the time of service unless other arrangements have been made. We accept the following payment methods: ACH, HSA, FSA, credit card.

If you have insurance (based on the program opted into), we will submit claims on your behalf, but it is your responsibility to ensure that any remaining balances, including copays or uncovered services, are paid promptly. Please contact us with any questions about your billing or coverage.

5. Medical Advice Disclaimer

The medical care and advice provided by Monarch Ideal Care is based on the specific information you provide and the results of your consultations. It is not a substitute for emergency medical care. For emergencies, please call 911 or go to the nearest emergency room immediately.

6. Confidentiality and Privacy

We are committed to protecting your privacy and confidentiality. Your medical information will only be used and disclosed in accordance with applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA). For more details, please refer to our Privacy Policy.

7. Termination of Services

Monarch Ideal Care reserves the right to terminate services if:

You repeatedly fail to attend or cancel appointments without proper notice.

You engage in abusive, threatening, or inappropriate behavior toward staff or other patients.

You fail to comply with medical advice or prescribed treatments.

You do not pay outstanding balances for services rendered.

We will provide reasonable notice in such cases, allowing you time to seek care elsewhere if necessary.

8. Changes to the Terms

We may update these Terms of Service from time to time. Any changes will be posted on our website and will be effective as of the date indicated. We encourage you to review these terms periodically to stay informed of any updates.
